Indications/Uses
Schizophrenia. Short-term treatment of acute manic episode associated w/ bipolar I disorder. Prevention of manic, mixed or depressive episodes recurrence in bipolar I disorder.
Dosage/Direction for Use
Starting dose: Schizophrenia 10 mg daily. Manic episode 15 mg daily as single dose or 10 mg daily in combination therapy. Preventing recurrence 10 mg daily. Maintenance dose: 5-20 mg daily. Elderly ≥65 yr, renal &/or hepatic impairment 5 mg daily.
Administration
May be taken with or without food: Orodispersible tab: Place tab in mouth to rapidly disperse & swallow w/ or w/o water. Alternatively, tab may be dispersed in a full glass of water or other suitable beverage (orange juice/apple juice/milk/coffee) immediately before administration.
Contraindications
Hypersensitivity. Known risk for narrow-angle glaucoma.
Special Precautions
Not to be used for dementia-related psychosis &/or behavioural disturbances or treatment of dopamine agonist associated psychosis in Parkinson's. NMS; hyperglycaemia, diabetes, lipid alterations; prostatic hypertrophy, paralytic ileus; elevated ALT &/or AST, hepatic impairment, limited hepatic functional reserve; low leukocyte/neutrophil counts. Medicines increasing QTc interval, long QT syndrome, CHF, heart hypertrophy, hypokalaemia, hypomagnesaemia; thromboembolism; seizures; tardive dyskinesia; postural hypotension; sudden cardiac death; phenylketonurics. Galactose intolerance, Lapp lactase deficiency, glucose-galactose malabsorption. Avoid abrupt discontinuation. Combination w/ other CNS drugs, direct & indirect dopamine agonists; alcohol. May affect ability to drive & use machines. Pregnancy & lactation. Not for use in childn & adolescents.
Adverse Reactions
Wt gain; somnolence; orthostatic hypotension; elevated plasma prolactin levels. Eosinophilia, leukopenia, neutropenia; elevated cholesterol, glucose & triglyceride levels, glucosuria, increased appetite; dizziness, akathisia, parkinsonism, dyskinesia; mild, transient anticholinergic effects ie, constipation & dry mouth; transient asymptomatic elevations of liver enzymes; rash; arthralgia; erectile dysfunction, decreased libido in males & females; asthenia, fatigue, oedema, pyrexia; increased alkaline phosphatase, high creatinine phosphokinase, γ-glutamyltransferase & uric acid.
Drug Interactions
CYP1A2 inducers/inhibitors. Induced metabolism by smoking & carbamazepine. Metabolism inhibition by fluvoxamine. Reduced bioavailability by activated charcoal. Antagonized effects of direct & indirect dopamine agonists. Alcohol & medicinal products causing CNS depression. Concomitant use w/ anti-Parkinsonian & QTc interval increasing medicines.
